Ebola Splits Into Three Rails as Uganda Declares Its Outbreak Over

Three separate Ebola developments, each on its own trajectory, converged in the same week to reshape the global response to the virus's Sudan strain [1].

Uganda declared an end to its 2025-26 Ebola outbreak on Tuesday, 42 days after the last confirmed case — the internationally recognised threshold for outbreak closure [2]. The Ministry of Health confirmed 14 confirmed cases and four deaths, with all survivors cleared through post-recovery follow-up. The outbreak, concentrated in Kampala and surrounding districts, was contained through contact tracing and ring vaccination using the Merck rVSV-ZEBOV vaccine.

A fractional-dose approach from Hilleman could stretch the same vials to cover more people. An mRNA vaccine from Moderna could, in principle, be manufactured faster and at greater scale.

"The mRNA platform changes the economics of outbreak response," said Dr. Michael Schill, a vaccine economist at the Graduate Institute in Geneva who was not involved in any of the three developments. "You're not waiting 18 months for egg-based production. You can retool a facility in weeks."

The timing matters for the WHO's post-outbreak review process, which has been criticised for leaving African countries dependent on donated doses rather than independent manufacturing. Uganda's outbreak was contained with Merck doses shipped through the WHO's Emergency Use Listing pathway. Hilleman's trial is designed to produce data that could support African-manufactured alternatives. Moderna's authorisation adds a third manufacturing route, though the company has no announced plans for African production.

Uganda's Ministry of Health said the country would maintain surveillance capacity and stockpile vaccines for rapid deployment. The 42-day countdown to outbreak closure began on June 17, when the last confirmed patient tested negative for a second time [2].

For the Sudan ebolavirus strain, which has no approved vaccine before this week, the convergence of three independent pathways represents a shift from the emergency-driven model of the 2014-16 West Africa crisis, when vaccine candidates arrived too late for the worst of the outbreak. The infrastructure built since then — the WHO's R&D Blueprint, CEPI's vaccine platform, and the growing regulatory capacity of African authorities — means the next Sudan ebolavirus outbreak will be met with tools that did not exist a decade ago.
