A four-hospital study in Atlanta found no measurable link between rising hand-hygiene compliance and rates of drug-resistant infections, despite hand hygiene improving substantially during the study period. [1]
Emory University School of Medicine researchers used an automated electronic monitoring system — which records whether a health worker dispensed sanitizer after entering a patient's room — to track more than 22.7 million hand-hygiene opportunities from January 2021 through September 2022. Compliance rose from a monthly average of 41 percent to 57 percent across the units studied. [1]
Over that same window, the hospitals recorded 1,847 detections of multidrug-resistant organisms and other significant pathogens across 752,341 patient-days. Researchers found no clear inverse relationship between higher hand-hygiene adherence and lower detection rates at the unit level — a rate ratio of 1.01, essentially flat. [1]
The authors were careful not to overreach. "The interplay between hand hygiene, infection prevention efforts, and MDRO acquisition is complex," they wrote, "and an absence of proof does not negate the importance of hand hygiene." [1] The study's real target is methodology: most prior research linking hand hygiene to infection rates relied on direct observation, which studies show tends to overestimate compliance because workers behave differently when watched. Automated systems were built to fix that blind spot — and this one still couldn't find the effect everyone assumes is there.
