The signal is real. The assay is the question. Tuesday's first read on the aging-monocyte signature blood test for cognitive depression named the underlying biology. The Wednesday follow-on names what the test would actually measure.

Three meta-analyses converge on the structural finding. Across 27 studies and 2,277 patients, depression is associated with a standardized mean difference of 0.60 in absolute monocyte counts versus controls — a moderate effect with substantial heterogeneity [1]. A 2022 Nature Molecular Psychiatry meta-analysis of 104 studies pegged the monocyte SMD at 0.32 [2]. Berlin-based work in antidepressant-free patients sharpens it further: the shift is specifically toward non-classical monocytes, with reduced glucocorticoid receptor and GILZ expression in CD14+ cells but not in T cells [3].

That last finding is what makes the assay plausible and risky. The signal is real and cell-specific. But monocyte-to-lymphocyte ratio elevations track inflammation across cardiovascular disease, cancer, and autoimmunity. A diagnostic assay that confuses depression with chronic inflammation is a treatment-stratification tool dressed as a screening test. The validation cohort question — antidepressant-naive, age-matched, free of metabolic comorbidity — is the gate.

Service psychiatry's near-term use case looks more modest than the headline: identifying inflammation-related depression subtypes for trial stratification. Which is exactly what immunopsychiatry has wanted for fifteen years. The press release is bigger than the assay. The assay may still matter.
